Do not substitute live Git or file reads for captured evidence. Later worktree edits, unrelated paths, HEAD or index changes do not invalidate a published review. Each reviewer submits explicit whole-group review_complete receipts, including zero findings; only the creator finalizes after all coverage receipts. A revision_conflict from a failed storage flow requires a fresh tool invocation and re-reading current scope; do not retry within that failed flow. Creating scopes, recording findings and submitting receipts write private review records and require Code mode. read_review_change is read-only and remains available for evidence gathering in Plan mode. A review request does not authorize edits, commits or publication. Read-only child roles run in Plan mode: use them only for evidence gathering, then independently verify and record coverage in the authorized parent. Do not ask a read-only child to submit private review records or change its permissions.",
