RTK Hooks for Codex

Requires rtk on PATH and Node.js 22+. The PreToolUse hook adds an RTK suggestion for recognized commands without changing approval decisions or executing the command. Review and trust the hook in Codex after installation. RTK missing/errors/timeouts fail open. DSH post-tool output replacement is not included; run RTK wrappers for filtered output.
