Hyreflow plugin

The installable plugin package: four Agent Skills (skills/), an approval hook (hooks/) that auto-approves only read-only, non-spending hyreflow CLI commands, and the manifests each harness reads (.codex-plugin/, .claude-plugin/, .cursor-plugin/, plugin.json).
