Captures a decision as a lightweight document in memory/decisions/: the diagnosis, the decision, alternatives rejected and why, the validation plan, and the revisit-by date. Use after a non-trivial decision in a project without a chronicle/ layer. If the project has chronicle/ use chronicle-record, the strict-schema successor; to read prior decisions back use memory-query.
